Background
Pragmatic trials provide real-world evidence that can be used to make clinical decisions. Many RCTs that do not meet the requirements for pragmatism but contain features in opposition to pragmatism, have been published in journals of different types and incorrectly labeled as pragmatic. The development of a PRECIS-2 tool that can provide an objective and standardized assessment of pragmatic features is a good start.
Methods
In a pragmatic study it is the intention to inform policy or clinical decisions by demonstrating how the intervention can be implemented into routine care. Explanatory trials test hypotheses concerning the cause-effect relationship within idealised conditions. In this way, pragmatic trials could have lower internal validity than studies that explain and are more susceptible to biases in their design analysis, conduct, and design. A typical feature of pragmatic research is that researchers attempt to make their findings more meaningful by studying subgroups within the trial sample. This can result in unbalanced analyses that have lower statistical power. This increases the risk of omitting or ignoring differences in the primary outcomes. In the instance of the pragmatic trials included in this meta-analysis, this was a significant problem because the secondary outcomes weren’t adjusted for the differences in baseline covariates.
Furthermore, pragmatic studies can pose difficulties in the collection and interpretation of safety data. It is because adverse events tend to be self-reported, and therefore are prone to errors, delays or coding differences. It is crucial to improve the quality and accuracy of the outcomes in these trials.
Results
Although the definition of pragmatism may not mean that trials must be 100 percent pragmatic, there are benefits to including pragmatic components in clinical trials. These include:

These terms could indicate a greater appreciation of pragmatism in abstracts and titles, but it’s not clear whether this is reflected in the content.
Conclusions
In recent years, pragmatic trials have been gaining popularity in research as the value of real-world evidence is increasingly recognized. They are randomized trials that evaluate real-world alternatives to experimental treatments in development. They are conducted with populations of patients more closely resembling those treated in regular medical care. This approach has the potential to overcome limitations of observational studies that are prone to biases that arise from relying on volunteers, and the limited accessibility and coding flexibility in national registry systems.
Pragmatic trials offer other advantages, including the ability to use existing data sources and a higher chance of detecting significant differences than traditional trials. However, these trials could have some limitations that limit their credibility and generalizability. For example, participation rates in some trials might be lower than anticipated due to the healthy-volunteer influence and incentives to pay or compete for participants from other research studies (e.g., industry trials). The requirement to recruit participants in a timely fashion also restricts the sample size and the impact of many pragmatic trials. In addition certain pragmatic trials lack controls to ensure that the observed differences are not due to biases in the conduct of trials.
The authors of the Pragmatic Free Trial Meta identified RCTs that were published between 2022 and 2022 that self-described themselves as pragmatic. They evaluated pragmatism using the PRECIS-2 tool that includes the eligibility criteria for domains as well as recruitment, flexibility in intervention adherence and follow-up. They found 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or above) in at least one of these domains.
Trials with high pragmatism scores are likely to have broader criteria for eligibility than traditional RCTs. They also contain patients from a variety of hospitals. The authors argue that these characteristics can help make pragmatic trials more effective and relevant to everyday clinical practice, however they don’t necessarily mean that a pragmatic trial is completely free of bias. The pragmatism principle is not a fixed characteristic; a pragmatic test that does not have all the characteristics of an explanatory study can still produce reliable and beneficial results.
